OFFICE FINANCIAL POLICY
Dental treatment is an excellent investment in your overall health and wellbeing. Financial considerations should not be an obstacle to obtaining this important health service. The following payment options are available:
VISA, MASTERCARD, DISCOVER, AMERICAN EXPRESS
Regardless of the option chosen, you are fully responsible for payment of all procedures performed in this office. Account balances extending beyond 60 days from treatment date will incur finance charges at the rate of 12% annually or 1% monthly.
Senior citizens (age 65 and over) will receive a 5% cash courtesy for payment in full at the time of treatment with the exception of payment by Care Credit. A 50% deposit is required when reserving appointment time for lengthy and extensive procedures, as well as those involving lab time and expense.
RESERVATIONS/BROKEN APPOINTMENT POLICY
Our office provides several courtesies to help you keep your appointments. We make every effort to confirm your RESERVATION 24-48 hours before your appointment. If we are unable to reach you, we ask that you contact us at least 24 hours in advance to confirm your reservation or let us know if you are not going to be able to keep your appointment. Emergency situations will be evaluated on an individual basis. There will be no charge for the first cancellation or “no show” without 24 hour notification. The second will result in a $50.00 charge. The third may result in dismissal from the practice. We make every effort to respect your time. We ask that you please respect ours.
RETURNED CHECK POLICY
Returned checks will not be presented to the bank a second time. A $25.00 fee will be charged.
Our experienced staff will file your insurance claim and help you maximize your benefits. We ask that you take care of any deductible and other amount not covered by your policy at the time treatment is rendered.
